    Client Health History Forms

    Is there a standardized client health history form that Peak Pilates professionals use prior to meeting with a new client? Thanks!

    There are many methods and criteria that Pilates professional utilize to adequately assess a client and their needs upon program intake. Our Master Training Team has used several versions of their own in the past and as a fitness professional and fitness manager, I have personally researched and developed several forms over the years. Our Education Team has discussed making a standard intake form available, but we have not finalized plans for this project and hesitate at standardizing a form that may not meet the needs of all trainers and insight confusion surrounding intake guidelines, etc. Client intake and individual needs can be a delicate process, but a standard Health History form could be a project within this cope that is acceptable for all. I know that some trainers go as far as evealuations of posture, etc. and it is important to note that not all Pilates professionals have the specific knoweldge, skills and abilities to perform such analyses.
